By submitting the intake form, the veterinary clinic acknowledges and agrees to the following:

1. Client Authorization:
The clinic confirms that the pet owner has been informed of and consents to the selected aftercare services provided by Hereafter Pets, including cremation or aquamation as applicable.

2. Information Accuracy:
The clinic affirms that all information submitted on behalf of the pet owner is accurate and complete to the best of their knowledge.

3. Consent to Share Information:
The clinic has obtained the pet owner’s permission to share their personal and pet-related information with Hereafter Pets for the purpose of facilitating aftercare services.

4. Disclosure of Service Type:
The clinic has explained the difference between communal and private services (including the return or non-return of ashes), and has confirmed the pet owner has selected their preferred option.

5. Memorial Items:
The clinic has informed the pet owner of any requested memorial items, and that these items will be created and provided by Hereafter Pets.

6. Payment Responsibility:
The clinic acknowledges that payment responsibilities have been clearly explained to the pet owner, including whether payment is due to the clinic or directly to Hereafter Pets.

7. Privacy and Confidentiality:
The clinic understands that all shared information will be handled with confidentiality in accordance with applicable privacy laws and will only be used for the purpose of providing aftercare services.

8. Liability:
The clinic agrees that Hereafter Pets will handle the pet with respect and dignity at all times, and releases Hereafter Pets from liability arising from the transfer of inaccurate or incomplete information unless caused by gross negligence.
